Instructions on Completing the online form

Important PLEASE READ THESE INSTRUCTIONS CAREFULLY BEFORE COMPLETING THE FORM.

This form is for candidates who offer the Polytechnic Diploma in Electrical / Mechanical & Manufacturing / Chemical Engineering or equivalent qualification.

Candidates in full-time National Service should apply only on completion of such service.

Application Procedure	Application is to be made online at the following website: http://btech.eng.nus.edu.sg Please print a set of the completed form and submit together with the supporting documents and application fee (with a copy of the receipt) to: Bachelor of Technology Programme The National University of Singapore Engineering Blk EA #07-32 9 Engineering Drive 1 Singapore 117576
Application Fee	The Application Fee of $20 can be paid either directly at the Office of Finance located in University Hall at NUS, or through a cheque or bank draft made payable to "The National University of Singapore". Cheques and bank drafts should have the applicant's name, address and contact numbers written on the reverse side. Crossed personal cheques issued by Singapore banks are accepted.
Closing Date	The closing date for the submission of applications online is 17 August 2004, 3pm (Mechanical/Manufacturing Engineering) XX XXXX 2005, 3pm (Electrical Engineering) XX XXXX 2005, 3pm (Chemical Engineering)
Inaccurate and false information	Any inaccurate, false or incomplete information will render your application invalid.  Persons admitted on the basis of such information may be expelled or required to withdraw from the University.
